Physical Therapist - PRN

PACE@Home is seeking a Physical Therapist to evaluate and reassess participants upon referral from multidisciplinary team and to assess mobility level and other functional abilities. Provides and recommends therapeutic exercise, gait training, ambulation, etc. Plans and develops group exercise programs for maintenance therapy for participants.

Employee Type:  Full Time

Minimum Experience: At least one year experience as an PT preferred.  Minimum one year experience working with the frail or elderly required.  Must be licensed in PT in North Carolina, valid driver’s license and reliable transportation.  

Required Degree: Licensed to practice PT in North Carolina. Must maintain CPR and First Aid certification.

Other Requirements:  Must have Medical form filled out and signed by physician, PA or NP prior to working.  

Travel: 50%
