Eligibility & Cost


Eligibility

To become a PACE@Home participant, residents must meet the following conditions:

  • Aged 55 and older

  • Determined by the state of North Carolina to be in need of nursing home care

  • Have the ability to live safely in the community at the time of enrollment with PACE@Home service

  • Reside in the PACE@Home service area within the zip codes listed below:

    o    Alexander County (28601, 28630, 28636, 28678, 28681)

    o    Burke County (28601, 28602, 28612, 28619, 28628, 28637, 28655, 28666, 28671, 28680, 28690)

    o    Caldwell County (28601, 28630, 28633, 28638, 28645, 28667)

    o    Catawba County (28037, 28168, 28601, 28602, 28609, 28610, 28612, 28613, 28650, 28658, 28673, 28682)

    o    Lincoln County (28037, 28092, 28168, 28650, 28673)

Payment for Services

PACE@Home is funded by Medicare, Medicaid, and private payment to deliver necessary care. If you are eligible for Long-term care Medicaid, PACE services may be offered free of charge. If your income is over the threshold determined by Medicaid, you may still be eligible to participate by paying a premium or a Patient Monthly Liability.

As a PACE@Home participant, all health care services are provided or arranged by your PACE care team (except for emergency services). A participant may be fully liable for the cost of medical services received from an out-of-network provider or made without prior authorization (except for emergency services).
