Social Work Director

PACE@Home is seeking a Social Work Director to be responsible for the development and direction of the Social Work Department. This includes identification, developing and coordinating clinical and social support programmatic needs.

Employee Type:  Full-Time

Minimum Experience: Must have two years experience in a health related - medical setting and one year experience working with the frail and elderly population.

 Required Degree: Master’s Degree from an accredited School of Social Work.  Must maintain CPR and First Aid Certification.

 Other Requirements:  Must have Medical form filled out and signed by physician, PA or NP prior to working.
